Orthopedic Associates of SW Ohio participates with most health insurance carriers.

Billing Policy
We will bill your primary health insurance following each appointment and file a claim with your secondary insurance, if applicable. Any balance not covered by your insurance carrier(s) is your responsibility.

Co-payments
If your health insurance carrier requires that you make a specified co-payment for each office visit, it must be paid at the time of the visit.

Referrals
Some health insurance providers require that you have a referral from your primary physician before being seen by an orthopedic specialist. In some cases, we must have the referral in our office before we can treat you. Please make sure you are familiar with your provider's referral policies before making an appointment with us.
